Registered Nurse Case Manager (RN)

Coordinate patient hospitalization from pre-admission through post-discharge by collaborating with physicians, nurses, social workers, and other health team members to expedite medically appropriate, cost-effective care. Apply clinical expertise and medical appropriateness criteria to resource utilization and discharge planning to achieve desired health maintenance and disease prevention outcomes. Advise the health care team, provide leadership as necessary, and communicate inter-departmentally to improve clinical management across the continuum. Core processes include risk assessment, management of clinical populations, and utilization management.
