Registered Nurse Case Manager (RN)

Job Summary
Provide 1:1 holistic hospice nursing in patients' homes as a Registered Nurse Case Manager; partner with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy. Responsibilities include complete initial and ongoing assessments, administering medications and treatments, educating patients and caregivers on care techniques, and rotating on-call duties. Must have reliable transportation and a valid driver's license. Schedule is 8:00am–5:00pm, full-time with on-call expectations. Location: Nixa, MO (US).
Required Qualifications
- RN license (licensure to practice as a Registered Nurse)
- Assoc, Bachelor’s, or Master’s degree in nursing
- 1+ years of recent acute care experience (hospice/home health preferred)
- Reliable transportation with valid driver’s license and proof of auto insurance
- Ability to complete assessments and develop individualized care plans
- On-call duties rotation
