Registered Nurse Case Manager (RN)

Conduct 1:1 holistic support for patients and families in their homes by completing initial and ongoing comprehensive assessments to determine hospice needs. Develop individualized care plans with patients and families, update as needs evolve, administer prescribed medications and treatments, and educate caregivers on care techniques, medications, diet, and symptom management. Partner with an interdisciplinary team to guide end-of-life conversations while rotating on-call duties to ensure seamless care transitions. Requires 1+ years of acute care experience with valid driver's license and reliable transportation.
