Job Summary
As a Registered Nurse Case Manager, deliver in-home hospice nursing care as part of an interdisciplinary team. Responsibilities include completing initial and ongoing comprehensive assessments to determine hospice needs; developing individualized care plans with the patient and family; administering medications and treatments as prescribed; educating patients and caregivers on care techniques, medications, diet, and symptom management; leading meaningful end-of-life conversations; coordinating care transitions; rotating on-call duties; and ensuring compassionate, autonomous clinical practice with reliable transportation for home visits.
Required Qualifications
- Associates, Bachelors, or Masters of an accredited nursing program (RN)
