Job Summary
Registered Nurse Case Manager focused on in-home hospice care. Deliver individualized, holistic patient care by developing personalized care plans with patients and families, lead meaningful end-of-life conversations, and ensure symptom management with empathy and clinical excellence. Responsibilities include administering medications and treatments, educating patients and caregivers on care techniques, coordinating with the care team, rotating on-call duties, and facilitating seamless transitions of care. Requires RN with an accredited nursing degree and 1+ years of acute care experience; travel to patient homes in Sedalia, MO. Schedule is 8:00am–5:00pm, full-time with on-call expectations; benefits include health/dental/vision, life insurance, disability, 401(k), tuition reimbursement, PTO, and holidays.
Required Qualifications
- Associates, Bachelors, or Masters of an accredited nursing program (RN)
- 1+ years of recent acute care experience required; hospice/home health experience preferred
- Must have reliable transportation, valid driver’s license, and proof of auto insurance
