Job Summary
Registered Nurse Case Manager (RN) for Hospice Care providing skilled nursing in home or living facility; conducts assessments, plans, implements, and evaluates interdisiplinary care; delivers direct hospice nursing care to maintain comfort and quality of life; educates patients/family, coordinates care with outside agencies, ensures supplies and equipment, supervises Home Health Aide activities, and provides on-call coverage.
Required Qualifications
- Graduate of an accredited school of nursing and current Registered Nurse (RN) licensure in the State of Michigan
- Two years of recent clinical experience preferred; recent nursing school graduates encouraged to apply
- Homecare or hospice experience desired
