Registered Nurse Case Manager (RN)

Registered Nurse Case Manager (RNCM) in home-based care leading and coordinating patient care. You will provide one-on-one clinical care under the direction of the physician, educate patients and families, ensure safety, and manage care within the scope of practice. Major duties include planning individualized care through assessments (OASIS when applicable), coordinating multidisciplinary care, supervising nurse aides/LPNs, reducing patient risk, and ensuring smooth transitions and continuity of care. You will work autonomously with a focus on compassionate communication, attention to detail, and adherence to agency policies and Medicare/Federal guidelines. The role emphasizes visit-based care, home visits, and collaboration with an interdisciplinary team to improve patient outcomes in a post-acute/home health setting.
