Job Summary
As a Registered Nurse Case Manager (RNCM), you will provide one-on-one patient care and coordinate care as directed by attending physicians. Responsibilities include planning individual care plans, assessing patient conditions, administering medications, and supervising care provided by nurse aides and LPNs. You will also educate patients and families on the disease processes while ensuring safety and continuity of care. Key skills include attention to detail, patience, and strong communication abilities. You will work independently with the support of an interdisciplinary team to promote positive patient outcomes.
Required Qualifications
- Registered Nurse with current license in the state of employment
- Minimum one-year experience as an RN in an acute care setting
- Valid driver's license and auto insurance in your name as a driver
- Capable of all physical demands
