Provide Registered Nurse Case Manager services in patients' homes, collaborating with a care team to develop personalized plans of care, lead meaningful end-of-life conversations, and ensure symptom management with empathy and clinical excellence. Responsibilities include completing initial and ongoing comprehensive assessments, developing and updating individualized care plans with patients and families, administering medications and treatments as prescribed, educating patients and caregivers on care techniques, medications, diet, and symptom management, rotating on-call duties, ensuring seamless care transitions, and coordinating transportation needs as required.
