Job Summary
Registered Nurse Case Manager (RN CM) with PACE by IIH delivering in-home, participant-centered care. Responsible for assessing participants’ physical and mental wellness, coordinating and monitoring care, conducting home care nursing assessments, developing and implementing care plans, documenting regulatory assessments, and providing nursing interventions. Supervises Personal Care Assistants and LVNs in participants’ homes, records progress and scheduling, and ensures smooth care transitions with the IDT. Provides health education and counseling to participants and caregivers, supports end-of-life care coordination, assists with enrollment in the program, and maintains comprehensive documentation. Requires strong clinical nursing skills in home health, geriatrics, care planning, utilization management, and quality improvement, with the ability to work across center, hospital, nursing facility, and participant home settings. Must have a California RN license, CPR/First Aid, and be able to travel locally; BSN preferred. Required Qualifications
- Graduate of accredited nursing program
- Current California Registered Nurses license
- CPR certification with First Aid Certification
- Bachelor of Science in Nursing preferred
