Job Summary
Manage an assigned caseload of complex patients in a value-based care environment, driving reduced admissions, readmissions, and medical utilization. Accountable for panel metric performance in admission prevention, readmission prevention, and transitions of care. Own overall care coordination as the primary point of contact, ensuring alignment across the care team. Lead in-person interdisciplinary care planning meetings and perform timely nursing assessments for chronic condition management. Collaborate with primary care providers, social workers, and behavioral health specialists to evaluate needs and ensure care plan progression. Document visits in the electronic health record and provide patient education while advocating for individual goals. Requires current RN license, CCM certification, and 6-8 years of nursing experience.
Required Qualifications
- Current RN license in assigned state
- Bachelor degree in nursing
- Minimum of 6-8 years nursing experience
- Certified Case Manager (CCM)
- 2+ years experience in transitional nursing, emergency room nursing, care coordination, discharge planning, or home health
- US work authorization
- Access to reliable transportation and ability to travel throughout the communities OSH serves
- Ability to lift 50 lbs
Desired Qualifications
- Certified Case Manager (CCM) obtained within 12 months of hire
- 2-3 years of relevant care/case management experience
- Demonstrated skill in motivational interviewing, patient activation, time management, and navigating community and social resources
- Flexible and positive attitude
- Comfort with ambiguity and change
- High emotional intelligence as evidenced by ability to evaluate/perceive a situation from multiple lenses and understand various perspectives in coming to problem resolution
