Job Summary
Provide skilled nursing care, including medication administration and treatments, for hospice patients in home, assisted living, and skilled nursing facility settings. Conduct comprehensive assessments, coordinate care plans with caregivers, and monitor physical and emotional status to report changes to supervisors and physicians. Document all clinical activities, time records, and care updates in the computer-based system while participating in weekly interdisciplinary team meetings. Submit required documentation within specified timeframes and provide input for hospice aide evaluations. Weekend and holiday coverage is required as needed.
Required Qualifications
- Graduate of an accredited RN program
- Registered Nurse Licensed in the state of employment
- At least 1-2 years of RN experience
