Job Summary
Assess participants' needs and develop individualized plans of care through initial and routine re-evaluation assessments. Collaborate with families and multidisciplinary teams to coordinate nursing interventions, administer medications, and teach safe home care practices. Lead licensed and non-professional staff in delivering total patient care to frail elders with complex needs, while monitoring outcomes and revising care plans based on status changes. Maintain accurate documentation of functional status, care delivery, and PACE data. Rotating on-call shifts required for after-hours medical assistance via phone triage or home visits.
Required Qualifications
- Registered Nurse
- current Michigan licensure
- Must meet a standardized set of competencies (approved by CMS) before working independently
- Must have one (1) year of experience with a frail or elderly population
- current State of Michigan driver's license
- acceptable driving record
- Must be medically cleared for communicable diseases
- all immunizations up-to-date
- Must have reliable transportation available on a daily basis
- Must be able to lift forty (40) pounds or more
