Job Summary
Registered Nurse Case Manager for hospice home health. Provide end-of-life care and compassionate support to patients and families in private residences or assisted living/skilled nursing facilities. Maintains autonomy to make bedside care decisions, serves as a hospice educator, and collaborates with an interdisciplinary team during weekly office visits. Typical workload includes 4-6 patient visits daily and weekly program-office meetings with social workers, physicians, chaplains, and nurses. Requires an active RN license in the program state, at least two years of nursing experience (hospice/home health/community health in last five years), and reliable transportation. Location: Wauwatosa, WI (USA). EOE/AA M/F/D/V.
Required Qualifications
- Currently licensed to practice nursing in the state where the VITAS program is located.
- A minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
