Job Summary
Coordinate services and develop individualized care plans as an RN Case Manager to support patients and families through their care journey. Responsibilities include guiding patients through orientation and discharge criteria, coordinating education for patients/families, developing treatment plans, completing psychosocial assessments and discharge planning evaluations, and collaborating with the Rehab Program Director and interdisciplinary team to ensure safe transitions and high-quality care. Requires RN with an Associate Degree or RN Diploma and 2 years of experience. Location: Round Rock, Texas. Travel is not required.
