Job Summary
Registered Nurse Case Manager for Home Health Care. Responsible for intake health assessments, supervisory visits, and developing personalized care plans. Educates patients and families on healthcare techniques, disease management, and self-care. Collaborates with healthcare professionals to ensure coordinated, holistic patient care and maintains accurate EMR documentation. Monitors patient conditions, reports changes, and makes recommendations. Provides emotional support and creates a safe home environment. Per diem schedule with flexible hours; requires MN RN license and at least 1 year of relevant clinical experience. Location in the St. Paul/Oakdale area with driving to patient homes.
Required Qualifications
- Graduate of an accredited school of nursing
- Valid unencumbered RN license in the state of Minnesota
- Minimum of 1 year of experience in home healthcare, hospital, or similar clinical settings
- Ability to work independently, manage time effectively, and prioritize patient care needs
- Proficiency in using electronic medical record (EMR) systems
