Job Summary
Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice needs, utilizing the nursing process to write and initiate plans of care. Re-evaluate patient status regularly, revising care plans as necessary while administering prescribed medications and treatments. Counsel patients and families on nursing needs, self-care techniques, and medication instructions, assisting with activities of daily living to facilitate self-sufficiency. Coordinate care by communicating with physicians, community health personnel, and the interdisciplinary group, while maintaining accurate clinical notes and pain management records. Participate in on-call duties, supervise ancillary personnel, and attend quality assessment performance improvement teams.
Required Qualifications
- Graduate of an accredited school of nursing
- Current licensure in State
- CPR certification
- Bachelor's degree
- Licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Physical ability to lift, position, and/or transfer patients
- Physical ability to lift supplies and equipment
- Visual acuity and hearing to perform required nursing skills
- Ability to sit or stand frequently
- Ability to lift and/or move up to 50 pounds
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem-solving skills
- Basic math skills
- Nursing skills per competency checklist
- Ability to supervise hospice aides
Desired Qualifications
- One (1) to two (2) years of recent acute care experience in an institutional setting
- One (1) to two (2) years of recent experience in home care
- One (1) year of hospice care experience
