Job Summary
Plan, organize, and direct hospice care as a Registered Nurse Case Manager. Conduct comprehensive patient and family assessments, develop and revise care plans based on nursing diagnosis and physician orders, administer medications as prescribed, provide patient/family education, and coordinate care with the physician and interdisciplinary team. Participate in on-call duties, supervise hospice aides as needed, and engage in ongoing professional development and quality improvement activities. Requires current nursing licensure, CPR, and a Bachelor's degree is preferred; 1-2 years of hospice or home care experience is also preferred.
Required Qualifications
- Graduate of an accredited school of nursing
- Current licensure in State
- CPR certification
- Bachelor’s degree preferred
- 1-2 years of hospice care/home care experience preferred
- excellent communication skills
- ability to lift/move up to 50 pounds
- valid driver’s license with insured vehicle
