Job Summary
The Registered Nurse Case Manager plans, organizes, and directs hospice care, building from community resources to meet the needs of patients and families in homes and communities. Responsibilities include comprehensive patient and family assessments, developing and revising plans of care, administering medications as prescribed, coordinating care with the interdisciplinary team, educating patients and families, supervising ancillary staff, participating in on-call duties, and maintaining accurate clinical notes. Requires RN licensure, CPR certification, and a Bachelor's degree is preferred; 1-2 years of recent acute care and home care experience is preferred.
Required Qualifications
- Graduate of an accredited school of nursing
- Current licensure in State
- CPR certification
- Bachelor’s degree preferred
- 1-2 years of recent acute care experience in institutional setting and 1-2 years in home care preferred
- Excellent communication skills
- Ability to supervise hospice aides
- Ability to lift up to 50 pounds
- Valid driver's license with insured automobile
