Job Summary
Registered Nurse Case Manager plans, directs hospice care and builds from community resources to meet patient and family needs in homes and communities. Eligible for a $5,000 sign-on bonus. Responsibilities include comprehensive patient/family assessments, writing and initiating plans of care, regularly re-evaluating needs, administering prescribed medications, counseling patients and families, coordinating patient care as Case Manager with the Clinical Supervisor, maintaining accurate clinical notes, coordinating with physicians and the interdisciplinary team, providing health instructions, ensuring safety, supervising ancillary personnel, participating in on-call duties, and pursuing ongoing professional development.
Required Qualifications
- Graduate of an accredited school of nursing
- Current licensure in State
- CPR certification
