Job Summary
Registered Nurse Case Manager for Home Health in Loganville, GA coordinates patient care across the interdisciplinary team, develops and updates individualized plans of care, provides patient and family education on disease processes and self-management, completes required documentation (OASIS, visit notes), communicates patient status changes, connects patients with community resources, supervises LPNs and HHAs, participates in SOC/ROC conference calls, and maintains competencies through ongoing education. Requires GA nursing licensure, CPR, a valid driver’s license, and prior home health experience; offers a full-time schedule with rotating on-call weekend/holidays and a comprehensive benefits package.
Required Qualifications
- Graduated from an accredited nursing program (Diploma, Associate, or BSN preferred)
- Hold current licensure by the Georgia State Board of Nursing and valid CPR certification
- Possess a valid driver’s license, reliable transportation, and current auto insurance
- Have prior home health experience if applying as PRN staff
- Demonstrate knowledge of Medicare, Medicaid, and funding source regulations as applied to home health care
- Exhibit advanced clinical nursing skills and comfort collaborating with interdisciplinary teams
- Have proven decision-making, time management, and interpersonal skills, functioning independently in a home environment
- Are proficient with computers and basic technology for documentation, email, HR systems, and online training
