Job Summary
Deliver compassionate, comprehensive hospice nursing care as an RN Case Manager, including thorough patient assessments, individualized care planning, medication/treatment management, and family education. Collaborate with an interdisciplinary team to ensure coordinated end-of-life care, maintain accurate patient records, and provide on-call support. Role involves travel up to 100% within the assigned geography and coverage for Newburyport, MA, with potential support for New Hampshire and Maine as needed. MatrixCare power user proficiency and experience with home health/hospice settings are advantageous.
Required Qualifications
- Current and active RN license in the state of Ohio
- Bachelor’s degree in nursing (BSN) preferred
- Minimum of 2 years of experience in hospice
- In-depth understanding of end-of-life care principles and practices and certified home health
- Exceptional communication and interpersonal skills
- Ability to travel up to 100% of the time within the assigned geography
- Matrix and Enclara experience helpful
- Basic knowledge of iOs operating system
