Lead a care team to deliver best-in-class hospice care; coordinate with the Clinical Director and Medical Director to ensure high-quality patient and family experiences; present updates at IDT every two weeks; drive regulatory compliance and professional standards; maintain strong internal and external communication to achieve top CAHPS scores; collaborate with team to provide a concierge-level hospice experience; travel within your local market to serve Plano/Carrollton and surrounding areas.
Required Qualifications
- Must be a graduate of an approved school of professional nursing.
- Must have at least 2 years of nursing experience in the Hospice service line in the state of Texas.
