Job Summary
Assess and monitor patients' physical, emotional, and psychological needs while creating care plans aligned with their wishes. Administer medications, treatments, and interventions to manage pain and symptoms, then educate and support families and caregivers. Collaborate with the interdisciplinary team and maintain accurate, timely documentation. Participate in on-call rotation as required by the local branch. Work Mon-Fri from 8:00 AM to 5:00 PM in Forest Lake, MN, providing expert, patient-centered hospice care at home.
Required Qualifications
- Graduate of an accredited nursing school
- Current licensure to practice in state of operation
- One year nursing experience
- Licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
Desired Qualifications
- 3+ years nursing experience
- Experience in hospice or a similar setting
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
