Registered Nurse Case Manager - Hospice

Assess and monitor physical, emotional, and psychological needs of patients, then create hospice care plans aligned with their wishes and goals. Direct nursing care by administering medications, treatments, and interventions while providing pain and symptom management. Educate and support the patient's family and caregivers, collaborate with the interdisciplinary team, and maintain accurate, timely documentation.
