Job Summary
Home Health Registered Nurse (RN) responsible for supporting patient recovery at home, enabling patients to stay at home and spend time with loved ones. Educates patients and families on disease management, provides case management and clinical oversight to an interdisciplinary team. Requires RN license in the state you work, two years of RN experience (home health experience preferred), valid driver’s license, and current CPR/BCLS certification.
