Job Summary
Registered Nurse Case Manager Home Health role with CarePartners/HCA Healthcare in western North Carolina. Responsible for assessing home care patients’ physical, psychosocial, and environmental needs; completing OASIS and visit paperwork; coordinating with physicians and the inter-disciplinary team; communicating significant findings; supervising home health aides and LPNs/LVNs to ensure quality and continuity of services; participating in on-call rotation and emergencies. Qualifications include RN licensure, Associate Degree, and Basic Cardiac Life Support within 30 days of start date; valid driver's license.
