Registered Nurse Case Manager Home Health

Registered Nurse Case Manager Home Health coordinates patient-centered nursing care in patients' homes, manages a clinical panel, conducts comprehensive in-home assessments, develops and implements individualized care plans, and coordinates services across a provider network. Collaborates with physicians, social workers, therapists, and other team members to support timely transitions of care, prevent avoidable hospitalizations, and optimize health outcomes. Performs skilled nursing visits, monitors changes in condition, and educates patients and families on disease management and medications. Documents all care in electronic health records, identifies gaps in care, and communicates proactively with providers to maintain continuity and quality of care. The ideal candidate is a self-directed RN able to prioritize a dynamic caseload, with compassionate, culturally sensitive care and strong assessment and communication skills. This role serves as a clinical liaison between members, caregivers, and the network, contributing to efficient healthcare delivery and a mission-driven organization serving Connecticut residents.
