Registered Nurse Case Manager Home Health Brevard

Assess home care patients identifying physical, psychosocial, and environmental needs through documentation, clinical records, case conferences, and on-site evaluations. Complete OASIS, assessment, and visit paperwork while communicating significant findings, problems, and changes to the Clinical Manager and physician. Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services. Participate in on-call rotation and emergency calls according to agency policy. Collaborate with the interdisciplinary team to establish patient goals and maintain high-quality care under physician orders and Clinical Manager supervision.
