Job Summary
Provide comprehensive assessments of clients' health and social needs; develop and implement individualized care plans in collaboration with healthcare teams; monitor client progress and adjust care plans as necessary; provide education and resources to clients and their families; facilitate communication between clients, families, and healthcare providers; document all interactions and maintain accurate client records; advocate for clients to ensure they receive appropriate services and support; stay updated on best practices and regulations in case management and nursing.
Required Qualifications
- Registered Nurse (RN) license in the state of Indiana
- Minimum of 2 years of nursing experience, preferably in case management
- Strong assessment, communication, and organizational skills
- Experience with electronic health records (EHR) systems
