Job Summary
Coordinate the delivery of appropriate and timely patient care within the nursing department. Actively manage patients, collaborate with medical providers and interdisciplinary teams to establish individualized medical goals, and coordinate multi-disciplinary care conferences for high-risk or complex patients. Facilitate patient referrals and track progress/outcomes; perform staff nursing duties including assessment, documentation, education, and medication administration. Serve as the primary link for case managers and discharge coordinators in care coordination. Manage Specialty Clinic referrals/waitlists and serve as primary nurse for specialty visits; oversee OB Case Management intake, tracking, education, outreach, and postpartum follow-up. Coordinate with VTC/ETT with specialty providers and regional facilities; participate in triage and respond to emergency situations.
Required Qualifications
- Graduate of accredited nursing school; BSN preferred
- Current full unrestricted RN license in Alaska or another U.S. state (must apply for AK license before start)
- Basic Life Support (BLS) - American Heart Association
- National Case Management certification preferred
- Clinical Competency required within 3 months of hire and every three years
- 1+ year general nursing with 6 months at SEARHC outpatient clinic, OR
- 2+ years general nursing with minimum 1 year in ambulatory setting or 1 year case management experience
- BLS (AHA) | Clinical Competency Assessment | RN License - State of Alaska
