Registered Nurse - Case Manager

Job Summary
Assess patient needs and develop, implement, coordinate, and monitor individualized care plans across inpatient, outpatient, community, and telephonic settings. Coordinate treatment plans including preventive, therapeutic, rehabilitative, and psychosocial interventions while supporting continuity of care and follow-up following discharge. Collaborate with physicians, social workers, and community organizations to identify healthcare resources and provide health education to patients and caregivers. Maintain accurate documentation and assist in developing clinical guidelines and case-management processes. Participate in quality-improvement activities and administrative functions. Full-time role Monday through Friday with no on-call requirements.
Required Qualifications
- U.S. Citizen, U.S. national, lawful permanent resident, or individual otherwise authorized to work in the United States
- Associate's Degree of Nursing
- Current, full, active, unrestricted Registered Nurse license in any U.S. state, territory, or the District of Columbia
- At least one year of professional nursing experience following graduation
- Current Basic Life Support (BLS) certification (or willing to obtain)
- Demonstrated clinical competency within the previous two years
- Case Management Certification
