Registered Nurse Case Manager Full Time - Kennebunk / Wells / Sanford, Maine

Provide skilled nursing care, administer medications per physician plans, and perform comprehensive assessments for hospice patients in home, assisted living, and skilled nursing facility settings. Coordinate care plans with all disciplines, family, and caregivers while observing and reporting physical, emotional, and psychosocial status changes to supervisors and physicians. Participate in weekly Interdisciplinary Team Meetings and maintain all clinical documentation and time records through the computer-based system. Submit required information within specified timeframes and provide input to the Home Health Aide Supervisor for aide evaluations. Weekend and holiday coverage is required as needed.
