Registered Nurse Case Manager Full Time 10K Sign on Bonus

Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, then translate findings into patient-centered care plans. Deliver individualized, one-on-one nursing care including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate and empower patients and caregivers through clear instruction on symptom and medication management. Monitor patient progress closely, identify condition changes early, and communicate effectively with physicians and the interdisciplinary team to adjust plans and prevent hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation supporting quality and compliance. Work Monday through Friday with a weekend on-call rotation every third month in Brown Deer, Bayside, River Hills, Fox Point, Glendale, Whitefish Bay, and Shorewood communities.
