Job Summary
Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Deliver individualized, one-on-one nursing care including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate and empower patients and caregivers through clear instruction, while monitoring progress closely to communicate effectively with physicians and adjust plans to prevent hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation supporting quality and compliance. Work Monday through Friday with a weekend on-call rotation in the Menomonee Falls, Brookfield, Germantown, and Richfield communities.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Patient-focused mindset
- Organized mindset
- Motivated by delivering high-quality care
- Current driver's license
- Reliable transportation
- Auto insurance
- CPR certification (or ability to obtain certification)
- Ability to comply with required vaccinations and testing
