Job Summary
Registered Nurse Case Manager for flexible home care role. Responsibilities include conducting in-home client assessments and supervisory visits, developing and updating individualized care plans, supervising CNAs and caregivers, ensuring high-quality and compliant care delivery, collaborating with families and interdisciplinary teams, addressing client concerns with professionalism, and maintaining accurate documentation. Requires active Maryland RN license, CPR certification, at least 1 year of nursing experience (home care or case management preferred), strong assessment and care planning skills, ability to work independently with a flexible caseload, reliable transportation for travel to client homes, and excellent communication and leadership abilities. Flexible schedule and independent field work with an established agency.
Required Qualifications
- Active RN license in Maryland
- CPR certification
- At least 1 year of nursing experience (home care or case management preferred)
- Strong assessment and care planning skills
- Ability to work independently and manage a flexible caseload
- Reliable transportation to travel to client homes
