Job Summary
Registered Nurse Case Manager (field-based in Essex and surrounding counties, NJ) conducts in-person home visits, develops individualized care plans, and coordinates services for members enrolled in MLTSS/D-SNP programs with up to 75% travel. Responsibilities include care planning, collaboration with providers and interdisciplinary teams, documentation in electronic health records, and advocacy for safe transitions and appropriate services. Requires an active NJ RN license, Associate degree in Nursing, NJ Choice Certification, and 3+ years of clinical experience; residency in Essex or adjacent counties; travel up to 75%. Mon-Fri, 8:00 a.m.–5:00 p.m. EST; travel and home visits across Essex and surrounding counties.
Required Qualifications
- Active, unrestricted RN license in the state of New Jersey
- Minimum of 3 years of clinical experience in settings such as hospitals, home health, or ambulatory care
- Residency in Essex or adjacent counties
- Ability and willingness to travel up to 75% within Ocean and surrounding counties
- Successful completion of NJ Choice Certification, including: Exam score of 80% or higher
- Completion of state training modules
- Field mentoring, as required by the NJ Division of Aging Services
