Job Summary
Conduct comprehensive case management services through patient assessment, care planning, coordination, implementation, monitoring, and evaluation to promote continuity of care and optimal health outcomes. Develop and implement local strategies utilizing inpatient, outpatient, onsite, and telephonic case management while establishing mechanisms for proper implementation of patient treatment plans and follow-up care post-discharge in ambulatory and community healthcare settings. Create treatment plans that include preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure optimal wellness. Work onsite Monday through Friday from 7:00am to 4:30pm.
Required Qualifications
- Associate Degree in Nursing
- Basic Life Support (BLS)
- Certified Case Manager (CCM)
- Certified Disability Management Specialist (CDMS)
- Certified Rehabilitation Registered Nurse (CRRN)
- Certified Occupational Health Nurse (COHN)
- Certified Occupational Health Nurse-Specialist (COHN-S)
- Advanced Certification in Continuity of Care (ACCC)
- Certified Rehabilitation Counselor (CRC)
- Nurse Case Manager (RN-NCM)
- Care Manager Certified (CMC)
- Current, full, active, and unrestricted license to practice as a Registered Nurse
- Minimum of four (4) years of experience in discharge planning or case management
- Minimum of two (2) years of full-time experience in discharge planning or case management
- Full-time employment in a nursing field within the last 36 months
- U.S. Citizenship
