Job Summary
Deliver skilled nursing care as a Case Manager in Community Hospice, educate patients and families, and support end-of-life wishes on an interdisciplinary Home Health & Hospice team. Requires active Vermont RN licensure and 2 years of experience in home health, home hospice, or med surg (or a combination). On-call is not typically required and a $10,000 sign-on bonus is offered for full-time external applicants, with relocation assistance available. The role emphasizes autonomy and ongoing professional development through preceptorships and continuing education within a patient-centered home-based care philosophy.
Required Qualifications
- Current RN licensure recognized by the State of Vermont
- 2 years experience in either home health, home hospice, or med surg - or a combination of these three settings
