Job Summary
Conduct case management and perform home visits within Chittenden and Grand Isle counties to deliver skilled nursing care, educate patients and families, and create a supportive environment for end-of-life wishes. Maintain current RN licensure recognized by the State of Vermont and bring 2 years' experience in home health, hospice, or med surg settings. This in-person role offers autonomy and flexibility with a $10,000 sign-on bonus for full-time external applicants, supported by structured preceptorships and continuing education opportunities.
Required Qualifications
- Current RN licensure recognized by the State of Vermont
- 2 years' experience in either home health, home hospice, or med surg - or a combination of these three settings
- Appropriate experience in specific clinical area
