Job Summary
Conduct skilled nursing care and perform home visits within Chittenden and Grand Isle counties as a Registered Nurse Case Manager. Educate patients and families while building lasting connections to support end-of-life wishes in the comfort of their homes. Deliver interdisciplinary case management services with a focus on compassionate, expert care for individuals and families at every stage of life. Requires current Vermont RN licensure and two years of experience in home health, hospice, or med surg settings. Full-time position includes a $10,000 sign-on bonus for external applicants.
Required Qualifications
- Current RN licensure recognized by the State of Vermont
- 2 years' experience in either home health, home hospice, or med surg - or a combination of these three settings
- Appropriate experience in specific clinical area
