Job Summary
Deliver skilled nursing care and perform home visits within Chittenden and Grand Isle counties, while educating patients and families to support end-of-life wishes in their own homes. Manage case responsibilities as part of an interdisciplinary team, maintaining patient ratios and utilizing safe patient handling equipment. Requires current Vermont RN licensure and two years of experience in home health, hospice, or med surg. Full-time role with a $10,000 sign-on bonus for external applicants, including medical, dental, and vision benefits.
Required Qualifications
- Current RN licensure recognized by the State of Vermont
- 2 years' experience in either home health, home hospice, or med surg - or a combination of these three settings
- Appropriate experience in specific clinical area
