Job Summary
The Registered Nurse Case Manager is responsible for providing health assessments and managing the care of residents with disabling HIV/AIDS at Peter Claver Community. Duties include communication with primary care providers, supervising Certified Nursing Assistants, coordinating medication management, and ensuring compliance with California State Community Care Licensing regulations. Essential qualifications include a California Registered Nurse license, an Associate's degree, and strong experience in case management and crisis intervention skills. Candidates should also possess knowledge of HIV/AIDS, mental health issues, and harm reduction principles.
Required Qualifications
- California license as a Registered Nurse
- Required Associate's degree (A.A.) or equivalent from two-year college or technical school.
Desired Qualifications
- Preferred Bachelor's degree (B.A.) from four-year college or university.
- Preferred 2 to 5 years of experience in case management of HIV infected adults, knowledge of the disease, and experience with multiple diagnoses.
- Preferred 2 to 5 years of experience in assessment and strong crisis intervention skills, particularly with people with substance use, mental health, chronic illness, dementia and end of life issues.
- Preferred 2 to 5 years of experience in the ability to work with staff and clients around behavioral and psychiatric issues.
