The Hospice Registered Nurse Case Manager will coordinate hospice care, perform clinical evaluations, consult with physicians, and participate in care planning and assessments, primarily working with patients in their residences.
Required Qualifications
- Licensed Registered Nurse
- BLS Certified
- Valid Drivers License and Auto insurance Coverage
- Reliable transportation
- A minimum of 1 year of Clinical experience
