Job Summary
Manage all PACE participants in the ambulatory setting as the Registered Nurse, Case Manager & Care Coordinator Team Lead. Lead the team in delivering exceptional care to underserved communities while overseeing utilization and case management in both ambulatory and inpatient environments. Requires a current valid California RN license, one year of geriatric nursing experience, and one year in a managed care setting. BLS certification is mandatory. Starting salary is $104,000 annually with medical, dental, and vision insurance, 403(b) matching, and paid CME days. AltaMed serves over 400,000 patients across Los Angeles and Orange Counties.
Required Qualifications
- Current valid License as a Registered Nurse through the California Board of Registered Nursing
- Valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines
Desired Qualifications
- 1 year of experience in Geriatric Nursing
- 1 year of experience in a managed care setting, preferably in utilization or case management in an ambulatory or inpatient setting
