Registered Nurse Case Manager - Bilingual Spanish

RN Case Manager (bilingual Spanish) focused on care coordination for complex patients in a value-based care environment. The role is field-based and in-person, collaborating with PCPs and interdisciplinary teams to prevent avoidable admissions, drive efficient resource utilization, and ensure seamless transitions of care. Responsibilities include managing a caseload, achieving admissions/readmission/transitions metrics, leading in-person care planning meetings, conducting structured clinical assessments, documenting in electronic health records, providing patient education and empowerment, and advocating for patients and families. Requires strong motivational interviewing, patient activation, time management, and ability to navigate social resources; operates within Oak Street Health CVS Health ecosystem with remote-office hybrid opportunities as appropriate per the site. Fluency in Spanish or other community languages is required.
