Job Summary
Orchestrate the full interdisciplinary plan of care from admission through discharge to ensure a seamless patient experience. Design personalized care strategies that integrate clinical requirements with patient and family values and preferences. Monitor patient progress and oversee medical documentation to ensure treatment options and services align with diagnostic needs. Advocate for essential resources and proactively remove barriers to ensure the effective delivery of the health plan. Maintain continuous collaboration with supervisors and providers to optimize care implementation and post-discharge success. This Registered Nurse Case Manager role is part-time on the evening shift at Ascension St. Vincent's Riverside Hospital in Jacksonville, FL.
Required Qualifications
- Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required
- Diploma from an accredited school/college of nursing
- Required professional licensure at time of hire

- BLS Provider
- American Heart Association or American Red Cross accepted
