Registered Nurse – Case Manager

Job Summary
Registered Nurse case manager coordinates hospice care for home-care patients, develops and updates the plan of care in collaboration with patient and family, educates patients and families about the disease process, symptoms, medications, and care, oversees the interdisciplinary team, provides direct patient care, and participates in an on-call rotation. Responsibilities include applying the nursing process and Alive CARE visit framework to deliver safe, end-of-life care in patients' homes or other care settings, ensuring documentation, and supporting facility staff and caregivers.
Required Qualifications
- Current Tennessee RN License or compact license
- BSN preferred
- Minimum one-year nursing practice or equivalent experience
- Hospice, Oncology, or Home Health and/or long-term care preferred
