Job Summary
Coordinate care delivery for pediatric and internal medicine patients across active duty and family health clinics by assessing physical, emotional, and social status through direct communication and caregiver interviews. Develop patient-specific plans of care with short and long-term goals, collaborate with providers and community agencies to optimize resource access, and ensure timely referrals are ordered. Document all charting, coding, and tracking logs within the duty day while conducting standardized peer reviews and gap analyses as directed. Maintain a safe work environment by adhering to infection control policies and participating in interdisciplinary, facility, and Care Coordination meetings.
Required Qualifications
- Registered Nurse - Case Manager
- graduate from an associate (ADN) or baccalaureate degree (BSN) program in nursing accredited by a national nursing accrediting agency recognized by the US Department of Education
- Be eligible to obtain a national certification related to Case Management
- ADN will have a minimum of three years of experience (within the last four years) in full time clinical case management in a hospital or clinic with children, adults, families, seniors, or groups
- BSN will have a minimum of two years of experience (within the last four years) in full time clinical case management in a hospital or clinic with children, adults, families, seniors, or groups
- Basic Life Support
Desired Qualifications
- Familiarity with the military is a plus in understanding the needs of active duty patients
