Job Summary
Conduct comprehensive assessments of patients and families to identify hospice needs, including physical evaluations, symptom reviews, and medical history analysis. Perform hospice admissions and start-of-care visits, verifying eligibility, providing education on disease progression and medication safety, and completing all required documentation. Develop and update individualized plans of care based on patient goals and physician orders while administering prescribed medications and treatments. Serve as the primary Case Manager for assigned patients, coordinating services with social workers, chaplains, and hospice aides to ensure continuity of care. Provide ongoing pain and symptom management, adjust interventions as needed, and maintain accurate clinical documentation reflecting patient status and outcomes. Participate in on-call rotation and after-hours support as assigned.
Required Qualifications
- Graduate of an accredited nursing program
- Current, unrestricted Registered Nurse (RN) license in good standing
- Minimum of two (2) years of nursing experience
- Current CPR certification
- Valid driver's license
- Access to an insured, reliable vehicle

- Ability to use electronic devices (laptop, tablet, mobile phone) for documentation and communication in community settings
- Ability to manage the physical and emotional demands associated with hospice and end-of-life care
